Terms and Conditions, Assumption of Risk,
Waiver of Liability and Indemnification

Note to Parents and Guardians: This document includes legally binding clauses which restrict you and your child's ability to sue Hockey Calgary for damages if your child is injured while attending the Hockey Calgary event, and also includes an indemnification clause. Please read and understand the entire document before you sign it. If you do not understand it, please consult legal counsel before signing it. By signing this document, you accept it and agree to be bound by its terms.

MISCELLANEOUS

It is the responsibility of the parent / guardian to drop off and pick up a child on time as per the published schedule. Hockey Calgary does not accept responsibility for children who are left unattended beyond the hours listed in the published schedule AND WILL PROVIDE NOT SUPERVISION FOR SAME.

It is the participant's responsibility or their parents or guardians responsibility to ensure they have all equipment necessary to play ice hockey and that the equipment is in good certified condition and fits properly (Hockey Calgary adheres to Hockey Canada, Hockey Alberta, IIHF and CSA approved guidelines).

Hockey Calgary does not accept any responsibility or liability as a result of any injury which may result by using equipment that is not appropriately certified or in good condition, whether or not the defect is obvious or noticed by Hockey Calgary.

RIGHT TO DISMISS PARTICIPANT, SUPERVISION, AND USE OF PHOTOGRAPHS

Hockey Calgary's Event Director reserves the right to dismiss a participant when it is determined, at the Event Director's sole discretion, to be in the best interests of the child, other participants, and/or the event.

Hockey Calgary will provide appropriate supervision of the participants. By submitting this application, I hereby give my approval to child's participation in all activities of the Hockey Calgary event.

I also hereby authorize Hockey Calgary to use my child's photograph or video in any publicity or new releases including brochures, advertising in print or electronic format at Hockey Calgary's discretion.

I hereby certify that the applicant (my child) is in good physical shape and mental health. My child has no known medical conditions which might impact his or her ability to fully participate in the event.

ASSUMPTION OF RISK, WAIVER AND RELEASE OF LIABILITY, AND INDEMNIFICATION

I acknowledge on my behalf and on behalf of my child that there are risks inherent in playing game of hockey and in attending and participating in the Hockey Calgary event, including risk of serious bodily injury or even death. On behalf of myself and my child, I assume full responsibility for all legal risk and physical risk associated with participation in the Hockey Calgary event, whether such legal and physical risk results from bodily injury, death, property loss or damage, or any other loss or damage that might result from my child's participation in the Hockey Calgary event.

I, for myself and on behalf of my child, waive any and all claims against Hockey Calgary, its officers, directors, employees, successors, assigns, affiliates and partners (collectively, the "Releasees"), and forever releases and discharges the Releasees from any and all claims, demands, damages, liabilities, losses, costs, expenses, actions and manner of action with respect to personal injury, death, property damage or any other loss, no matter how or by whom so caused, including without limitation negligence on the part of the Releasees, which the participant may in any way suffer or incur as a result of attending and participating in the Hockey Calgary event.

I, for myself and on behalf of my child, agree to fully indemnify the Releasees for all claims that any other person may make against the Releasees arising through my child's participation in the Hockey Calgary event, including full indemnification for actual legal costs incurred in the defence of any such claim.
